Community Care Services Program (CCSP)
The CCSP is a Medicaid waiver program that provides a variety of community-based services. The goal is to assist individuals who are older and/or have disabilities with achieving safe, self-reliant lives. Additionally, the Health Force of Georgia is a state-approved participating provider of at-home care services. It serves adults who are eligible to receive the CCSP waiver.
Service Options Using Resources in a Community Environment (SOURCE)
SOURCE is a waiver program operated under Medicaid that offers assistance Georgia residents who are elderly, ill, or have disabilities. The care needs of individuals served under the program usually coincide with those receiving care at a nursing home. Eligible recipients can receive the requisite care in their homes or communities.
Georgia Pediatric Program (GAPP)
GAPP is a Medicaid program for medically qualified children under the age of 21. Children of families eligible for Medicaid can apply for GAPP at any time and must meet the same level of care necessary for admission to a hospital or skilled nursing facility. The Georgia Department of Community Health (DCH) manages GAPP.
